Puppy Inquiry Questionnaire

The use of this questionnaire helps my record keeping of Puppy Inquiries. If you are serious about owning a Bichon, please take a few minutes to answer these questions. If I should have puppies available there will be lots of follow up beyond these questions. Our much loved Bichon puppies will be placed in Approved homes only. Thanks, Sylvia
